Homeless Families Hub brings together evidence, practical resources and professional expertise to support better health outcomes for families experiencing homelessness. 

The hub has been developed by three professionals bringing together complementary expertise across inclusion health, public health nursing, frontline practice, strategic leadership, digital transformation and policy implementation.

Together, the authors combine strategic insight, clinical expertise and extensive experience working alongside people experiencing exclusion. This breadth of perspective has shaped a resource designed not simply to inform professionals, but to support consistent, confident and compassionate action.

Health Inequalities

Lisa is a Registered Nurse and Health Visitor with more than 30 years' experience in the UK. Originally trained in Australia, she founded and led Surrey's award-winning Health Inclusion Team, providing innovative outreach to marginalised and homeless families and tackling health inequalities.

As Surrey Health Lead for the national Families First Partnership Programme, Lisa leads health engagement in safeguarding reform across acute, community, mental health, primary care and maternity services. She works with partners to strengthen early intervention, family support and multi-agency child protection, improving outcomes for children and families.

A Queen's Nurse, Professional Nurse Advocate and Surrey Core20PLUS5 Ambassador, Lisa is recognised for her compassionate leadership, frontline expertise and commitment to partnership working with vulnerable families. Her team's work has received national recognition from the Burdett Trust and the CPHVA for Equality, Diversity and Inclusion.

Jane is a Registered General Nurse, Public Health Specialist and Queen's Nurse with more than 40 years' experience delivering and developing healthcare for people experiencing exclusion.

Jane has worked across a wide range of excluded communities and settings, from hostels and day centres to prisons and street-based services. She currently works as Health and Social Care Lead at Groundswell, combining extensive frontline experience with strategic expertise and a strong understanding of the barriers faced by people who experience exclusion from mainstream services.

Jane brings a unique combination of inclusion-health practice, public health expertise and strategic influence. Her experience spans service development, advocacy and working across professional and organisational boundaries, helping to translate the realities of exclusion into practical approaches that can improve how services respond.

Jane brings an important perspective to the Homeless Families Hub, helping ensure that the toolkit is grounded in evidence, informed by lived and frontline experience, and focused on what professionals and services can do differently to improve outcomes for families experiencing homelessness.

Anne brings extensive experience of working with complex issues and translating them into practical, structured solutions. She combines strategic thinking, stakeholder engagement and a strong understanding of how systems work in practice, bringing different perspectives together to identify opportunities for improvement and create approaches that are both meaningful and deliverable.

Her strength lies in making complexity accessible — turning ideas, evidence and experience into frameworks, tools and resources that enable people and organisations to work differently. She is particularly interested in challenges that cross organisational boundaries, where collaboration, innovation and a fresh perspective can unlock new solutions.

Anne is developing an ambitious vision for Homeless Families Hub as more than a resource. The longer-term ambition is to develop a roadmap towards more consistent assessment, better information sharing and more joined-up practice, using digital approaches to reduce silos and strengthen the skills and confidence of professionals. The aim is to help shape new ways of working that identify need earlier and improve outcomes for children and families.
